As part of the NHS England Additional Roles Reimbursement Scheme (ARRS), we are seeking a skilled and motivatedAdvanced Clinical Practitioner (ACP)to join our Primary Care Network (PCN) team.

The Advanced Clinical Practitioner (ACP) will be able to assess and treat varied clinical cases, having advanced skills to assess the patient with a differentiated or undifferentiated clinical diagnosis.

This role is based on 4 sessions, part time.


Main duties of the job

Advanced communication,problem-solving and decision-making skills will be used to initially assess andprovide on-going patient care. Working within local and nationally agreedprotocols and guidelines, the ACP exercises independent judgement to assess,investigate, diagnose, plan, implement and evaluate the clinical care andmanagement of patients.

The post holder will provide clinical expertise for developing andenhancing the service and will demonstrate advanced skills and competencies andensure that evidence-based practice is inherent in all aspects of care andtreatment.

Job responsibilities

Key Responsibilities:

Clinical Assessment and Care

* Provide a level of advanced andcomprehensive clinical assessment based on a specialist body of knowledge andadvanced clinical reasoning skills.
* To appropriately assess, examine,investigate, diagnose and treat a wide range of complex or urgent carepatients, resulting in the safe management and appropriate referral of patientswith undifferentiated, undiagnosed and complex presentations.
* Ensure patients are informed and consentis gained prior to treatment, investigation and management, as appropriate.
* Is able to identify the need forappropriate diagnostic tests and interpret the results.
* To identify and intervene proactivelywhere circumstances contribute to an unsafe environment for patients, staff orrelatives and escalate these appropriately.
* To share knowledge and expertise, actingas an expert resource to others across primary and secondary care, asappropriate.
* Contribute to the development andimprovement of systems and processes that facilitate patients being able toimprove their health.
* To improve the quality of the patientexperience by identifying and meeting the individual clinical needs ofpatients, including issues pertaining to child protection and vulnerableadults.
* Prescribe and review medication fortherapeutic effectiveness, appropriate to patient needs and in accordance withevidence-based practice and national and practice protocols, and within scopeof the PCN.
* To maintain up to date patient recordsin accordance with policy, including the use of the Electronic Patient RecordsSystem.
* Ability to adapt specialist clinicalknowledge and skills to different clinical settings and influence servicedelivery and patient care.
* Responsible for rapidly and continuouslyevaluating the patients condition and response to therapeutic interventionsand treatment modalities and modifies the plan of care for optimal patientcentred outcomes.
* Contribute to the review and developmentof clinical patient pathways to ensure a standardised approach to care.
* Actively implement effective systems andprocesses for infection prevention and control management and relevant auditwithin their clinical area.
* Educates patients, relatives and staffas necessary ensuring understanding has been interpreted accurately.
* Ensures patients views are consideredin the decision-making process.
* Participates in meeting and promotingthe health education needs of patients and carers.
* Promotes the health through co-ordinatedactivities and programmes with patients, colleagues and carers where appropriate.
* Practice autonomously as an ACP usingcollaborative and independent judgement to assess, investigate, diagnose, plan,implement, manage, and evaluate clinical nursing and medical care in themanagement of patient.
* Develop, implement, and evaluate patient management plans incollaboration with patients, their families, and the multidisciplinary team.
* Deliver safe, evidence-based, and patient-centred care, ensuringthat the individuals needs are met.
* Provide follow-up care and support, monitoring progress andadapting treatment plans where necessary.
* Provide clinical leadership and mentoring to junior colleagues andother members of the clinical team.
* Work closely with GPs, nurses, and other healthcare professionalsto ensure integrated care delivery.
* Promote best practices in clinical care and contribute tocontinuous professional development and team learning.

Quality and Compliance

* Participate in audits, reviews, and quality improvement initiativeswithin the practice.
* Ensure that all clinical activities comply with relevant policies,procedures, and regulations
* Maintain accurate and timely patient records in accordance withdata protection laws and practice protocols.
* Alert other team members to issues of quality and risk
* Assess own performance and take accountability for own actions,either directly or under supervision
* Contribute to the effectiveness of the team by reflecting on ownand team activities and making suggestions on ways to improve and enhance theteams performance
* Work effectively with line management to meet organisational need
* Effectively manage own time, workload and resources

Training and Development

* Stay up-to-date with the latest clinical guidelines, advancements,and best practices in primary care.
* Disseminate learning and information gained to other team membersin order to share good practice and inform others about current and futuredevelopments (e.g. courses and conferences).
* Assess own learning needs and undertake learning as appropriate

* Infection Prevention & Control

In accordance with the Health and Social Care Act 2008, the postholder will actively participate in the prevention and control of infectionwithin the capacity of their role. TheAct requires the post holder to attend infection prevention and controltraining on induction and at regular updates and to take responsibility for thepractical application of the training in the course of their work. Infectionprevention and control must be included in any personal development plan orappraisal.

Person Specification


Clinical Skills

* Competent in advanced clinical assessment and diagnostic skills
* Ability to manage and treat acute and chronic conditions
* Proficient in prescribing and clinical decision-making


Qualifications

* Registered Nurse (Adult)
* Advanced Clinical Practitioner (ACP) or Advanced Nurse Practitioner (ANP) qualification (Level 7 or equivalent)
* Relevant clinical postgraduate qualifications or experience


Knowledge and Expertise

* Sound understanding of national clinical guidelines and healthcare policies
* Knowledge of safeguarding, mental health, and long-term condition management


Qualities and Attributes

* Ability to work autonomously and manage a caseload
* Excellent communication and interpersonal skills
* Strong problem-solving skills and ability to prioritize effectively
* Commitment to ongoing professional development


Other

* Right to work in the UK
* Willingness to undertake additional training and development as required


Experience

* Extensive clinical experience within a primary care, urgent care, or equivalent setting
* Demonstrable experience in minor illness and chronic disease management
* Experience of working autonomously and within a multi-disciplinary team
* Leadership or mentoring experience
