Job Summary
We are redesigning our community services model and implementing a new Intermediate Care Team and therefore an exciting opportunity has arisen to appoint an Occupational Therapist on a 0.6 WTE basis to deliver urgent community response services. This role will play a crucial part in developing the service, providing essential support and care to individuals, with a focus on promoting independence, dignity, and well‑being.
Key Objectives
* Optimise safe and timely discharge from an inpatient stay for patients who need some form of continuing care or prevent admission with interventions that can be managed at home.
* Provide a high standard of Occupational Therapy intervention within the Intermediate Care Service across the Cwm Taf Morgannwg University Health Board (CTMUHB) localities, health board sites, various community settings and patient homes.
* Play an active role in the delivery of training programmes and, with appropriate training, lead on training sessions with delegated responsibility from the Senior Occupational Therapist/AHP Lead.
Responsibilities
* Autonomously work alongside members of the multidisciplinary team ensuring best current evidence‑based clinical management of patients referred within the defined specialist clinical area.
* Work as an autonomous practitioner with relevant post‑registration experience and undertake specialist clinical duties within a defined clinical area.
* Take delegated responsibility from the Senior Occupational Therapist/AHP Lead within the Intermediate Care Service managing patients at risk of unnecessary hospital discharge delay or admission, and be responsible for providing assessment and treatment plans.
* Manage own daily caseload, supervise generic Physiotherapy/Occupational Therapy Technicians and Health Care Support Workers, guide/mentor Band 5 Occupational Therapists and Occupational Therapy students.
* Provide regular specialist Occupational Therapy input to the Intermediate Care Service, including advice, equipment, treatment programmes and appropriate referrals.
* Ensure patient confidentiality, privacy and dignity at all times and communicate effectively with patients, parents, guardians and carers, adapting communication techniques as needed.
* Provide specialist assessments and advice on appropriate ADL equipment and other therapeutic equipment, liaise with and advise patients and carers for ongoing use in line with University Health Board Guidelines.
* Engage in regular communication with other Occupational Therapy team colleagues to ensure clinical practice is appropriate and informed.
* Represent the service at staff meetings, group meetings and patient case conferences to provide input regarding patients and give feedback to relevant staff.
* Maintain and develop current knowledge of evidence‑based practice in each area of rotation.
* Possess keyboard/IT skills to maintain personal and patient records, produce information sheets, participate in teaching/training sessions and access online information.
* Work within CTMUHB clinical guidelines and Royal College of Occupational Therapy guidelines, and maintain a good working knowledge of national and local standards, monitoring quality of practice.
* Maintain own clinical professional development (CPD) by keeping abreast of new trends and incorporating them into practice.
* Collect, analyse and present data in terms of service drivers, performance and future development goals.
* Plan and organise own caseload, balancing other patient‑related and professional demands.
* Formulate and deliver an individual Occupational Therapy intervention plan based on evidence‑based practice and clinical assessment.
* Recommend the best course of intervention and provide training to patients and carers in self‑management.
* Delegate work and responsibilities appropriately to healthcare support workers and other colleagues, and train other staff including healthcare, social care, care home and third sector staff.
* Maintain accurate, comprehensive and up‑to‑date patient clinical records in line with department requirements, HCPC standards and Royal College of Occupational Therapy Standards of Practice.
* Communicate assessment and treatment results to appropriate disciplines using letters, reports and verbal feedback, ensuring confidentiality.
* Participate in audit and collect appropriate data and statistics for the service.
* Take delegated responsibility from the senior Occupational Therapist/AHP Lead and manage own daily caseload, supervise Occupational Therapists and generic OT/PT Technicians and Occupational Therapy students.
* Be professionally and legally accountable for all aspects of own work, including management of patients in care.
Qualifications and Knowledge
* Degree/Diploma in Occupational Therapy.
* Professional registration recognised by HCPC.
* Sound knowledge of Standards of Professional Practice and Rules of Professional Conduct.
* Knowledge of a range of policies and procedures.
* Current in mandatory training requirements.
Desirable Qualifications
* Member of BAOT.
* Completion of a validated Clinical Educator's course.
* Evidence of attendance on appropriate courses to enhance specialist knowledge/skills.
* Knowledge of clinical guidelines and standards for falls (local and national).
Experience
* Varied and relevant experience within the core Band 5 rotation.
* Experience of community working.
* Experience of older adults.
* Experience of working in a multi‑disciplinary setting.
Desirable Experience
* Work within a multi‑disciplinary team setting, admission avoidance and fall prevention.
Skills and Attributes
* Good communicator.

* Welsh language speaking/listening skills at Level 3 or above.

* Successful DBS (Enhanced).
* Ability to carry out full range of duties of the post, including travel around various bases and on‑call duties.
