Overview
Location: Grantham, Lincolnshire. This is a new pilot service developed in partnership with Lincolnshire Partnership NHS Trust as part of the NHS Mental Health Pathway. The service will go live in October 2025 with a full training and induction programme for the team.
Role
Recovery Worker. You will work with individuals stepping down from acute mental health wards to support them to transition into the community. You will manage a small caseload, provide person-centred and recovery-focused support, and help with the day-to-day running of the service.
Responsibilities
* Provide recovery-focused support through key working sessions and group work.
* Develop individually tailored recovery and risk management plans with effective discharge planning.
* Help individuals gain skills and coping strategies to promote long-term recovery and independence.
* Work on a 24-hour rolling rota, including evenings, weekends and waking night shifts.
The Ideal Candidate
* Ideally experience in the mental health sector with a solid understanding of the recovery model.
* Solution-focused, person-centred approach; able to work autonomously and as part of a team.
* Capable of adapting to a fast-paced environment and challenging situations with empathy and patience.
