Key Responsibilities
* Provide specialist palliative care to patients with complex clinical and psychosocial needs
* Manage and coordinate a defined patient caseload, ensuring high-quality, individualized care
* Offer expert advice on pain and symptom control, including physical, emotional, and spiritual support
* Collaborate with multidisciplinary teams to plan, implement, and evaluate patient care
* Support patients and families in discussing and planning end-of-life care preferences
* Deliver education, mentoring, and training to healthcare staff to enhance palliative care practices
* Participate in clinical audits, quality improvement initiatives, and service development
* Act as a key link between hospital services and community-based care providers
Requirements & Qualifications
* Registered Nurse qualification with valid professional registration
* Bachelor's degree in Nursing (or equivalent)
* Postgraduate qualification in a relevant specialty (or willingness to complete)
* Significant post-registration clinical experience
* Proven experience in supportive and palliative care
* Strong communication, leadership, and decision-making skills
* Experience in teaching, mentoring, and knowledge sharing
* Ability to manage complex patient needs and challenging clinical situations
* Experience in audit, research, or quality improvement is an advantage
* Prescribing qualification (desirable but not essential)
