Overview
Locum Consultant in Elderly Care - Acute and Community Frailty – East Sussex Healthcare NHS Trust, Eastbourne District General Hospital. East Sussex Healthcare NHS Trust seeks an enthusiastic Consultant in Elderly Care to join the Acute Frailty Team at Eastbourne DGH. This role contributes to the development of integrated acute and community frailty services and supports high-quality care for older people across East Sussex.
The successful applicant will work as part of a multidisciplinary team delivering frailty services to a local population of approximately 544,000 people. The role focuses on early assessment and management of frailty, supporting rapid clinical decision-making and enabling safe, effective care pathways that promote independence and avoid unnecessary hospital admission where possible.
Responsibilities
* Provide consultant leadership in frailty assessment across clinical areas including the emergency department, clinical decision unit, acute medical unit, inpatient wards and the virtual frailty ward.
* Support community-based assessments and work closely with discharge teams and community services to facilitate safe transitions from hospital to home.
* Participate in multidisciplinary ward rounds and board rounds; deliver outpatient clinics aligned with the post holder's special interests.
* Support the development of surgical liaison services for frail surgical patients and assess patients referred by other specialties.
* Contribute to service improvement initiatives across the frailty pathway.
* Take part in the General Internal Medicine on-call rota with consultant colleagues.
* Engage in teaching, clinical governance, audit and quality improvement activities; opportunities to develop specialist interests and support research and education within the Trust and with regional academic partners.
* Help shape innovative frailty services and improve outcomes for older people across East Sussex within a supportive clinical team.
Qualifications and Experience
Essential
* MB BS or equivalent
* Membership or Fellowship of the Royal College of Physicians
* Full GMC registration with licence to practice
* Wide experience in all aspects of General (Internal) Medicine
Desirable
* MD or equivalent
* eSCE in acute Internal Medicine
* Experience in setting up an ambulatory care service or other relevant medical specialties
* Ability to develop a specialty or subspecialty interest within Acute Internal Medicine
Training & Experience
* Experience in general internal medicine; related courses and qualifications as appropriate
Administration & Management
* Evidence of participation in management; understanding of Clinical Governance and Appraisal
Audit, Research & Publications
* Understanding of medical audit, quality improvement and research; ability to lead clinical research projects
Personal Skills & Personal Attributes
* Effective communicator, able to work in a multi-disciplinary team
* Leadership qualities and ability to work with colleagues to develop local services
* Proficiency with information technology and Microsoft Office
* Good professional attendance and commitment to continuous improvement
Teaching
* Experience in teaching medical students, nursing staff and other professional groups; supportive of educational activities
