The Role
Are you passionate about delivering compassionate, person-centred care to older adults? University Hospital Wishaw is seeking dedicated and enthusiastic healthcare professionals to join our new dynamic Frailty Unit.
Our Frailty Unit provides specialist multidisciplinary care for older people living with frailty, focusing on early assessment, rehabilitation, and safe discharge planning whilst applying home first principles. We are proud to offer a supportive and innovative environment where staff are empowered to make a real difference in patients’ lives. You will work within a collaborative team, benefit from ongoing professional development, and contribute to service improvement initiatives that enhance patient outcomes.
You will contribute to the care of patients admitted to the Frailty Unit including provision of pharmaceutical care, medication advice and prescribing.
These exciting new roles will contribute to the seven-day Frailty Service to ensure patients receive high quality pharmaceutical care. You will become an integrated member of the consultant-led multidisciplinary team, liaising with primary care, hospital and community colleagues. There is a requirement for you to be an established practicing independent prescriber due to the complexities of patients presenting within frailty as both prescribing and de-prescribing will be an integral part of this role.
Full and part time hours will be considered.
