What you will do:
* Deliver a high-quality social work service, assessing the needs of older adults and their carers, and preparing personalised support plans.
* Conduct strengths-based Care Act assessments and Mental Capacity assessments, ensuring compliance with statutory guidelines.
* Foster partnerships with individuals and their families, prioritising their needs and promoting independence and wellbeing.
* Manage and prioritise caseloads effectively, meeting deadlines and adapting to changing demands.
* Contribute to safeguarding practices by carrying out Section 42 enquiries and participating in safeguarding meetings.
* Work collaboratively with multidisciplinary teams to address any challenges and ensure the safety and wellbeing of service users.
* Provide guidance and support to less experienced colleagues, promoting a culture of learning and excellence.
