As Lead PCN Mental Health Practitioner, you'll play a crucial role in transforming community mental health services. We're collaborating with the Integrated Care System to offer 'Place Based Care', ensuring communities receive fast and effective treatment in their GP surgeries.
Your role will include shaping the service while managing a small team of Mental Health Practitioners. You will set up and deliver patient clinics that provide support, medication advice (if qualified to prescribe), treatment, education, and problem-solving approaches to community members experiencing mental health challenges. You'll provide a dedicated service that ensures individual needs are met seamlessly.
We welcome full-time or part-time applications and are flexible to discuss different working patterns within GP practice hours.
Primary Care Networks (PCNs) are groups of GP practices working together in a local area. In Norwich, the largest PCN in Norfolk, it is divided into four PCNs with several GP practices. Your role will be based in Central Norwich.
As the Lead PCN Mental Health Practitioner, you will lead a small team and handle operational and clinical mental health issues in your GP practice(s). You will work autonomously, managing your clinics, providing interventions, and acting as a vital mental health resource for patients aged 16 and over. You will conduct Mental Health Assessments and serve as a conduit to specialist mental health services when needed.
The ideal candidate will be conscientious, kind, and compassionate, capable of working independently and as part of a team. You should have good organizational skills, system resource awareness, and experience in a mental health setting. You will be an autonomous practitioner, making clinical judgments and delivering high-quality care.
This role helps bridge gaps between Primary and Secondary healthcare services. You should be comfortable working collaboratively and building strong relationships with healthcare professionals across your network and within GP practices.
