Responsibilities
Conduct comprehensive assessments of clients to identify their needs and strengths.
Develop and implement individualized care plans in collaboration with clients and other healthcare professionals.
Advocate for clients' rights and access to appropriate services and resources.
Provide crisis intervention and emotional support to clients and their families.
Facilitate support groups and workshops that promote skill development and self-advocacy.
Maintain accurate and detailed records of client interactions and progress in compliance with legal and ethical standards.
Participate in interdisciplinary team meetings to ensure integrated care for clients.
Stay current with best practices and policies in social work and healthcare.
Requirements
A relevant social work degree and current registration with the appropriate regulatory body.
Proven experience working as a social worker in a healthcare or hospital setting.
Strong understanding of mental health, addiction, and social issues affecting clients.
Excellent communication and interpersonal skills for effective interaction with diverse populations.
Demonstrated ability to work collaboratively in a multidisciplinary team environment.
Strong case management and organizational skills to handle multiple cases effectively.
Commitment to continuous professional development and staying informed about social work practices.
Hours Per Week: 35.00
Start Time: 09:00
End Time: 17:00
Pay Per Hour: £30.15
Location: Swindon, Wiltshire
