Job summary

Whitby Coast and Moors Primary Care Network are looking to recruit an enthusiastic and compassionate Advanced Clinical Practitioner. Our PCN is a PCN Pilot site and is seeking an ACP to focus on acute presentations.

The PCN is working to increase same-day appointments to free up practices and improve continuity for complex patients. Additionally, it aims to develop on-the-day "surge" capacity to support practices during high demand or workforce reductions.

The post holder will provide urgent/on-the-day appointments for both paediatric and adult populations across four GP practices. You will be able to work independently, be an independent prescriber, and manage a wide range of acute conditions in children and adults.

Main duties of the job

1. Provide urgent/on-the-day appointments for paediatric and adult populations.
2. Work independently as an autonomous practitioner and prescriber.
3. Manage a clinical caseload, liaising with practices and Whitby Community Hospital UTC.
4. Engage with partners and stakeholders, leading joint work across health and social care sectors.
5. Support primary care locality groups in prevention and self-care initiatives.

Job responsibilities

Core Clinical Functions

* Provide compassionate, professional patient service, offering guidance and reassurance.
* Respect patient dignity, privacy, and confidentiality.
* Assist in treating acute patients.
* Manage a clinical caseload proactively, using analytical skills for differential diagnosis.
* Work autonomously, developing expertise within your specialty.
* Manage care pathways, adapting to patient needs to reduce unnecessary admissions.
* Advocate for patients, facilitating shared decision-making.
* Manage complex disease and symptom management, evaluate responses, and adapt treatments.
* Empower patients to self-manage conditions through education and risk stratification.
* Develop and deliver clinics aligned with national and local guidelines.
* Provide clinical advice for commissioning and act as a resource across services.
* Undertake clinical competencies to demonstrate expertise.
* Assess and manage psychological reactions to diagnosis and treatment.
* Maintain accurate documentation and records.


Management & Leadership

* Collaborate with partner agencies and influence strategies in health and personal care.
* Provide clinical leadership and act as a role model in holistic care.
* Supervise junior clinical team members and lead appraisals.
* Contribute to budget management and service performance review.
* Lead service redesign and improvement projects.
* Promote service improvement and lead on clinical governance initiatives.
* Support development of complex conditions and strategies.
* Represent the PCN in meetings at various levels.


Person Specification


Experience

Essential

* Extensive post-registration experience at band 7 or above within the specialty.
* Proven leadership skills.
* Experience in service improvement and managing change.
* Autonomous working experience in the specialty.

Desirable

* Experience in patient advocacy, research, audit, and evidence-based care.
* Stakeholder engagement and teaching experience.


Knowledge and Skills

Essential

* Advanced clinical, managerial, and leadership skills.
* Expert knowledge of diagnosis and management within the specialty.
* Effective planning, organizational, and communication skills.
* Ability to lead, influence, and make decisions confidently.
* Knowledge of current clinical guidelines and governance.
* Research and audit capabilities.
* IT proficiency and report writing skills.

Desirable

* Understanding of national specialty agendas.
* Experience in clinical and nursing research.


Qualifications

Essential

* Registered Practitioner (NMC or HPC).
* First degree in a related subject.
* Master’s degree or equivalent experience.
* Post-registration qualification in the specialty.
* Relevant clinical courses (e.g., advanced assessment, prescribing).
* CPD evidence.

Desirable

* Teaching qualification, leadership courses, advanced communication training.
