As our Lead Support Worker (known by us as Support Coordinator), you’ll support customers with complex needs in their homes and out in the community. You’ll be part of a central team that steps in when services need cover, so no two days are the same. What you’ll do
* Create person-centred support plans and lead a small team to deliver them
* Support customers with mental health, substance use, homelessness and isolation
* Help people build daily living skills and confidence in their community
* Carry out risk assessments, reviews and safeguarding checks
* Set up and end tenancies, making sure customers feel safe and supported

You have…
* Experience creating support plans and leading others to deliver them
* A calm, resilient approach and confidence working on your own
* Skills to assess referrals and coordinate support for complex needsA passion for helping people live their best life

Practical bits
* You’ll work Monday to Sunday on a rota-based system
* You’ll work 37.5 hours per week on a rota-based system
* You’ll cover a patch across services in West Yorkshire
* You need a vehicle insured for business use, and we pay your mileage
* You need an Enhanced with barring list DBS check that we pay for
