Role Details:
Our Urgent Community Response Support Workers are vital to healthcare teams in community settings and hospitals.
You will be supported through comprehensive training and induction, with a focus on learning and development.
The service provides urgent care support at home to prevent hospital admissions and facilitate discharges, operating 7 days a week across Sidmouth, Seaton, Axminster, and surrounding areas.
Previous community experience is preferred but not essential; training will be provided. You will work with multidisciplinary teams to respond to urgent needs, helping people stay at home or return from hospital quickly.
You will receive a thorough induction, including support for the National Care Certificate.
Qualifications include a Level 3 Diploma/NVQ 3 in Health and Social Care or equivalent, or relevant experience working with older adults in the community. IT skills and a valid driving license are essential.
Support is available for lone workers, and a caring, inclusive team environment is maintained.
Working Pattern: Various shifts, including early and late shifts, with at least alternate weekends. Specific hours will be discussed at interview.
