In this vital role, you’ll help ensure patients have a safe, smooth and well‑planned transition from hospital to home or onward care. Advert Coordinate timely and safe patient discharges by ensuring effective communication across the system. Update patient information accurately on Millennium to support discharge planning. Work collaboratively with the Multi‑Disciplinary Team to prepare clear and accurate referrals. Support a designated cluster of wards, assisting additional wards when needed to maintain optimal patient flow. Ensure smooth and consistent communication of discharge information between RUH wards and community partners. Attend key meetings, handovers and relevant training sessions. Handle patient information sensitively, ensuring all data—especially confidential information—is managed in line with Information Governance standards. Develop a strong understanding of discharge processes (e.g., transport arrangements) and how these link to electronic systems such as Millennium. Provide comprehensive administrative support to ward staff to facilitate efficient discharge processes. Detailed job description and main responsibilities To support the ward discharges by: 1. Coordinating communication across the system required to enable ward discharges in a timely and safe manner. 2. Updating Millennium, RUH IT system, with discharge related information eg patient Status, Patient Flow System. 3. Working Collaboratively with Multi-Disciplinary Team to prepared accurate referrals. 4. Working within your cluster of wards and being prepared to assist other wards as workloads require ensuring that flow is optimized at all times. 5. Communicating with patient and carers as required with regard to existing care arrangement and ongoing discharge planning. 6. Allow for the smooth communication of discharge information between RUH wards and community providers. 7. This role requires active listening skills and the ability to document accurately. 8. Ensuring you are professional and helpful to all callers at all times 9. Be aware of hospital pressures relating to flow and discharge. 10. Attending key meetings and training opportunities. 11. Exercise an appropriate level of decision making. 12. Ensuring that patient information, including that of a highly confidential nature, is shared appropriately and meets Information Governance standards. To be aware that breaches in this require reporting (via Datix) and to implement changes in the process if system issues are identified. 13. Maintaining a high standard of both verbal and written communication. 14. To have an in depth understanding of the processes required for ward discharges, for example transport etc and how this is then linked with electronic records (Millennium). 15. To demonstrate awareness of the specific discharge issues that affect smooth transition for patients. 16. Providing robust and accurate data. 17. To support fully the administrative role of the ward staff. To be aware of own development needs and to seek support as needed by: 1. Attending 1:1 teaching or training in groups, according to the need of the service, and in conjunction with the Clinical Lead. 2. Recognising training needs independently and acting on these, informally or formally as needed. 3. Following written and verbal training information on the processes involved in the administrative role. 4. To update staff in a timely way if/when changes occur. 5. Key elements of the training will include: o Using the relevant parts of Millennium and Patient Flow, including updating Patient Status o Accurate data input 6. Liaison with the Discharge Service and RUH site office. 7. Attending Discharge Co-ordinator team meetings whenever possible. 8. Develop working relationships with key people or teams, such as the Community Hubs Care Coordination Centre, Discharge Liaison Practitioners, Discharge Hub, Ward Managers, Therapy team leads, and ward discharge coordinators. Have a good knowledge of current computer packages used within the RUH and update skills when necessary. It may be necessary for the post holder to undertake additional tasks from time to facilitate the demands of the service.
