Main duties of the job
As a Staff Nurse you will be responsible for providing care topeople admitted to our ward for rehabilitation. We work closely as a multi-disciplinaryteam. Our focusispromoting the comfort and wellbeing of patients,enablingthemto live as independently as possible. Pershore is asmall but friendly team to work with. As a team we encourage ongoing learningand development through reflection and clinical supervision which we try andembed in our daily practice. Our nurses are required to facilitate the day today running of the ward, administering medications, working alongside all membersof the MDT as well as patients, their families and carers to provided 24/7 rehabilitationto enable the highest level of independence and quality of life. Our nurses arerequired to attend meetings as needed such as MDT and CHC or DischargePlanning.

Job description
Job responsibilities

5. Practice as a member of theCommunity Hospital Nursing team, to support and work in partnership with thewider multidisciplinary team.
6. Planning and Delivery ofIndividual Nursing Care under Direction of Senior Staff
7. Work within the NMC Code ofProfessional Conduct.
8. Assess, plan, and, implementand evaluate patient focused nursing care.
9. Document care given andmaintain contemporaneous records.
10. Maintain records in accordancewith the Trusts policy and that of NMC Code of Professional Conduct.
11. Assist in implementation ofhealth promotion strategies, as agreed by Ward Manager.
12. In consultation with seniorqualified staff, provide and monitor patient centred care within Trust agreedstandards.
13. In collaboration with otherteam members organise the reception and discharge of patients ensuring effectivecommunication in transfer of information.
14. Carry out delegated nursingcare.
15. Use and participate in research/ audit in care delivery.
16. Adhere to and promote allclinical practice, health and safety and corporate Trust Policies and Guidelines.
