On the back of a lot of change in the service, we are now recruiting for a Lead Support Worker, and you’ll be at the heart of supporting our customers who have come from an entrenched street homeless background to develop their skills to live more independently in the community.
You’ll make a big difference each day as you help our customers achieve their hopes and aspirations. Amazing, we know!
Typical day as a Support Coordinator (known as Lead Support Worker)
1. Creating support plans with your customers and coordinating our small team of Support Workers working to the plans.
2. Supporting working-age adults to maintain accommodation, move into secure and permanent housing, and address secondary needs such as substance misuse, mental health, and physical health conditions.
3. Helping customers with daily activities like making meals, shopping, budgeting, maintaining tenancy, and moving to independent living if able.
4. Carrying out risk assessments, support planning, goal setting, and reviews.

You bring
* Passion for supporting customers to live their best lives, with a collaborative approach and attention to detail.
* Experience in creating person-centered support plans and supporting colleagues to work to them.
* Experience in coordinating and assessing customer referrals.
* The ability to work on your own initiative, stay calm under pressure, and be resilient.
* Access to a car is helpful but not essential; a free staff parking is available.
