Trowbridge Primary Care Network (PCN) islooking for a dedicated, proactive and skilled dietitian to join ourexperienced, supportive and friendly multidisciplinary team. In this rewardingrole, you will provide expert nutrition and dietetic advice to patients of allages and from diverse backgrounds and settings.

Main duties of the job

You will provide expert nutrition and dietetic advice to patientsacross all age groups, working closely with multidisciplinary teams to support andpromote better health and wellbeing in the community. As a Trowbridge PrimaryCare Network clinician, you will deliver tailored dietary assessments andeducation to support patients with a range of diet-related health conditions. Whilstbeing based at our GP practices, your work will include patients from diversesettings, such as care homes, and will involve collaborative care planning toensure continuity across primary, community, and secondary care. You will playa central role in developing and improving local nutrition support services,supporting patient transitions from hospital to home, and advising onnutrition-related medication management. In addition, you will contribute toclinical governance, service evaluation, audit, and training to uphold highstandards of practice and drive continuous improvement.


About us

Since 2019, Trowbridge PCN has steadily grown to become a key partof community heathcare provision in Trowbridge, with a dedicated team ofaround 30 staff. The multidisciplinary team includes Clinical Pharmacists, CareCoordinators, and First Contact Physiotherapists, all working collaborativelyto improve the health and wellbeing of the Trowbridge population.

Operating across three main sitesTrowbridge Health Centre,Roundstone Surgery, and Wingfield Roadthe PCN has a friendly, supportiveatmosphere and a commitment to delivering high-quality, patient-centred care.The team is well-led by two experienced Clinical Directors, supported by thePCN Manager and Practice Managers.

Job responsibilities

Job Summary

Requirements for role

BSc or pre-reg MSc in Dietetics under a training programmeapproved by the British Dietetic Association

Registered member of the Health and Care Professionals Council(HCPC)

Role responsibilities:

Provide specialist nutrition and diet advise to patients, theircarers, and healthcare professionals through treatment, education plans andprescriptions.

Educate patients with diet-related disorders on how they canimprove their health and prevent disease by adopting healthier eating anddrinking habits.

Provide dietary support to patients of all ages (from early-lifeto end-of-life care) in a variety of settings.

Work as part of a multi-disciplinary team to gain patientscooperation and understanding in following recommended dietary treatments.

Develop, implement and evaluate a seamless nutrition supportservice across the PCN, working with community and secondary care whereappropriate, and aimed at continuously improving standards of patient care andwider multi-disciplinary team working.

Work with clinicians, multi-disciplinary team colleagues andexternal agencies to ensure the smooth transition of patients discharged fromhospital back into primary care, so that they an continue their diet plan.

Make recommendations to PCN staff regarding changes to medicationsfor the nutritional management of patients based on interpretation ofbiochemical, physiological and dietary requirements

Implement all aspects of effective clinical governance for ownpractice, including undertaking regular audit and evaluation, supervision andtraining.

Undertake a range of administrative tasks

Ensure delivery of best practice in clinical practice, caseloadmanagement, education, research, and audit, to achieve corporate PCN and localpopulation objectives.

Your responsibilities

To have greatattention to detail and be able to document patient records accurately.

In the performance of the duties outlined in this JobDescription, the post-holder may have access to confidential informationrelating to patients and their carers, Practice staff and other healthcareworkers. They may also have access to information relating to the Practice as abusiness organisation. All such information from any source is to be regardedas strictly confidential.

Information relating to patients, carers, colleagues, otherhealthcare workers or the business of the Practice may only be divulged toauthorised persons in accordance with the Practice policies and proceduresrelating to confidentiality and the protection of personal and sensitive data.

Tomonitor service provision to ensure that services are being delivered in linewith contract.
