RiversideHealth Partnership have an exciting opportunity for a motivated, enthusiastic and dedicatedClinicalPharmacist with Management responsibilities to lead an excellent team ofpharmacists, pharmacy technicians and medicine coordinators. This role will be anintegral part of the multi-disciplinary team at Riverside Health Partnership,driving positive changes in healthcare for local people. We arelooking for a professional who has a prescribing qualification and has provenmanagement experience with skills to lead a team.


Main duties of the job

To work as part of the managementteam in the practice and supportwith achieving efficient and effective prescribing activities. To promote highquality, cost effective and evidence-based use of medicines within primary careworking alongside the Nottinghamshire Integrated Care Partnership. To provide leadershipto the pharmacy and medicines management team including mentoring, support andprofessional development. Working as part of a multi-disciplinary team in apatient-facing role, to provide primary support to general practice staff withregards to prescription and medication queries. Ensuring that the practiceintegrates with community and hospital pharmacy to help utilise skill mix,improve patient outcomes.

Riverside Health Partnership is a 4 site practice with a patient population of just over 23,000. We are a forward thinking proactive practice in a semi-rural area.


Job responsibilities

Main Duties:

1.Toprovide leadership to the pharmacy and medicines management team includingmentoring, support and professional development.

2.Todevelop effective protocols for the team to ensure efficient and effectiveprocesses are in place for patients.

3.Towork as part of a multi-disciplinary team in a patient facing role toclinically assess and treat patients using their expert knowledge of medicinesfor specific disease areas.

4.Takeresponsibility for the care management of patients with chronic diseases andundertake clinical structured medication reviews toproactively manage people with complex polypharmacy.

5.Ensurepatients have appropriate monitoring tests in place when required

6.Providespecialist expertise in the use of medicines whilst helping to address both thepublic health and social care needs of patients and to help in tacklinginequalities.

7.Provideleadership on person centred medicines optimisation (including ensuringprescribers in the practice conserve antibiotics in line with localantimicrobial stewardship guidance) and quality improvement, whilstcontributing to the quality and outcomes framework and enhanced services.

8.Supportpatients to take their medications to get the best from them, reduce waste andpromote self-care, through structured medication reviews.

9.Supportthe integration of general practice with the wider healthcare teams (includingcommunity and hospital pharmacy) to help improve patient outcomes, ensurebetter access to healthcare and help manage general practice workload.

10.Toassist and lead where appropriate in identifying cost effective andevidence-based use of medicines.

11. To reconcile hospitaldischarge letters, ensuring that data provided is correct and updated on theclinical system.

12. To participate in prescribingaudits of prescribing in areas directed by the GPs, feedback the results andimplement changes in conjunction with the practice team

13. Answer medicine relatedenquiries from GPs, practice staff and patients. Suggesting and recommendingappropriate solutions and provide follow-up for patients to monitor the effectof any changes as indicated.

14. Analyse, interpret, andpresent medicines data to highlight issues and risks to support decision makingacross the primary care team

15. Contribute pharmaceuticaladvice for the development and implementation of new services that havemedicinal components (e.g. advice on treatment pathways and patient informationleaflets) including the development of the practice based dispensary.

16. To assist in reducingmedication errors such as Implement changes to medicines that result from MHRAalerts, product withdrawal and other local and national guidance.

17. Work with the general practiceteam to ensure the practice is compliant with CQC standards where medicines areinvolved, including regular audits.

18.Tocontribute to the PCN delivery ofthe highest possible quality standards.

19. To maintain computerisedrecords in accordance with data protection principles to ensure patientconfidentiality.

20. To keep up to date withcurrent pharmacy practice, new drugs, and their uses

21. To attend the practice meetingwhen required

22. To represent the practice atlocality Prescribing Leads Meetings with responsibility to inform thepartnership of actions.

23.Developrelationships and work closely with other pharmacy professionals across PCN andthe wider health and social care system.

24.Torespond to patient complaints in accordance with the practice complaintsprocedure.

25.To abide by the General Pharmaceutical Societys code ofethics, observing and applying all professional, ethical, and legal obligations

Management Duties:

1. To provide management and leadership to the pharmacy andmedicines management team including mentoring, support and professionaldevelopment.

2.Promote a healthy safe and effective working environmentthat provides a high-quality service to patients.

3.Always ensure adequate cover on a rota by suitablyqualified staff to provide a robust 5-day service.

4.Must be proactive in the future development of the teamand work alongside the management team to achieve the practices strategic aims.

5.Ensure regular audits of work across the team.

6.To work independently and manage own time effectively.

7.Assess own performance and take accountability for ownactions.

8.Co-ordinate and contribute to annual staff appraisals forthe pharmacy team.

9.To arrange regular meetings with the pharmacy and widerteam.

10. Workalongside the practice management team and attend meetings where required.

11. To be a partof all recruitment in the pharmacy team.

12. Ensure theteam are up to date in all areas of training.


Person Specification


Qualifications

* 1. Masters degree in pharmacy or equivalent
* 2. Postgraduate clinical qualification and/or significant relevant clinical experience to demonstrate similar level of knowledge
* 3. Registered with the General Pharmaceutical Council
* 4. Qualified prescriber
* 5.Minimum of 2 years experience as a pharmacist, in acute hospital, community pharmacy or community health services, demonstrated within a practice portfolio
* 6. Experience of managing a team
* 7. Experience of working with the public
* 8. Experience of being responsible and accountable for own professional actions, working independently within professional and organisational boundaries and being responsible for the actions of others within their area of responsibility
* 9. Experience in planning, developing and delivering pharmacy services
* 10. Excellent advanced theoretical and practical knowledge of PCNs, General Practice and evidence based medicine
* 11. Ability to analyse, interpret and evaluate specialist medicines related information or data.
* 12. Effectively manage own time, workload, and resources.
* 13. Ability to communicate sensitive and complex specialist medicines related information and advice to patients & carers, medical teams and other healthcare professionals.
* 1. Completed Centre for Pharmacy Postgraduate Education (CPPE) Clinical Pharmacist 18-month training pathway.
* 2. Experience of working within primary care
