Care Co-Ordinator - Community Wellbeing and Independence Team - Job Description

Grade 5 - £26,403 for 26.5 hours per week for 52.14 weeks per annum equating to £18,910.26 pro rata.

CWAIT (Community Wellbeing and Independence Team) supports adults in their own homes across Pembrokeshire. We provide high-quality, person-centred care to individuals with assessed needs, helping them live safely, independently, and work towards achieving their personal outcomes.

As a regulated service, governed by Care Inspectorate Wales (CIW), we are fully committed to meeting the standards set out in the Social Services and Well-being (Wales) Act 2014 and the Regulation and Inspection of Social Care (Wales) Act 2016.

We are currently offering an exciting opportunity for a Grade 5, Systems Co-ordinator position, working 26.5 hours per week over 5 days. This role will provide essential support to manage scheduling demands, ensure continuity of care, and maintain compliance with regulatory responsibilities.
