We are looking for an experienced and motivated leader to manage a busy, fast‑paced service that plays a vital role in supporting safe, timely and person‑centred transitions from hospital into the community. This is a pivotal leadership role, managing Adult Social Care practitioners working within the acute hospital setting and ensuring that high‑quality social care practice, safeguarding, and strengths‑based approaches are embedded throughout all aspects of the service. You will have a significant impact on outcomes for people by enabling coordinated, integrated support that promotes independence, choice and wellbeing. Working closely with Integrated Neighbourhood Team (INT) Managers, health partners and wider system stakeholders, you will champion collaborative working across hospital and community pathways. Your role and responsibilities As Team Manager, you will provide strong operational leadership within the hospital setting, overseeing complex decision‑making and ensuring safe, timely and effective discharge planning. You will use professional judgement, analytical skills and evidence‑based practice to respond to complex and high‑risk situations, balancing individual choice with safeguarding responsibilities. You will lead by example in applying professional ethics, legal frameworks and human rights principles, supporting consistent, transparent and accountable decision‑making. You will promote a culture that empowers individuals and families to exercise autonomy, choice and control, while ensuring appropriate protection for those at risk. A key part of the role is strengthening integration across health and social care pathways. You will build and maintain effective relationships with clinical teams, community services and voluntary sector partners, ensuring seamless collaboration across hospital and community services. You will also support performance management, quality assurance and workforce development, ensuring the delivery of safe, strengths‑based and person‑centred practice. Alongside this, you will contribute to service development, continuous improvement and innovation within the Hospital to Home pathway, helping to shape future ways of working.
