Overview

The South Glos. Recovery Team North is looking for a Specialist Recovery Nurse Practitioner to join our supportive and friendly team. The team is based in Thornbury with a geographical area covering South Gloucestershire, including Yate, Patchway and other suburban areas. This is a wide geographical area with limited access to public transport. The post offers the opportunity to develop your skills with new training in Structured Clinical Management (SCM) with a supervision program in place to support staff.


Responsibilities

* Hold a caseload as a Key Worker for service users under secondary mental health services and coordinate and deliver evidenced-based interventions, including commissioning packages under the Care Act.
* Welcome applications from qualified nurses with experience of working with people with acute and enduring mental health needs; recent graduates who have completed their preceptorship will be considered.
* Key work a varied caseload including a range of presentations and diagnoses; support clients through recovery, including stepping up and down care when needed.
* Cover duty and duty nurse responsibilities; work as an intervention-based service identifying service user needs and how they can meet their goals.
* Support the running of the clinic and medication management; work closely with social workers, the IST, psychological therapy service (PTS) and the primary care liaison team (PCLS).
* Contribute expertise within the MDT; develop, deliver and review comprehensive and complex treatment programmes in line with evidence-based practice; manage risk for service users with complex needs and carers, bringing in other resources as required.
* Act as key worker for service users with the most complex needs, providing defined interventions to individuals on other workers’ caseloads.
* Plan, deliver and evaluate defined, specialist therapeutic interventions as indicated in the assessment process, aligned with personal recovery plans, including service users on other caseloads.
* This may include: a) individual or group therapeutic interventions; b) psychological treatments such as CBT, DBT, family interventions; c) psychosocial interventions; d) motivational and coping enhancement strategies; e) medication management; f) interventions under the Mental Health Act.
* Deliver a range of activities/interventions to improve carers’ ability to support the service user and enable their relationship with the service user.
* Develop and maintain partnership working with other services across all treatment episodes, including liaison with Primary Health Care Team, inpatient and intensive services, day services, voluntary sector and carers/advocates.
* Collaborate with service users and carers to develop complex crisis plans, rapid access plans, advance statements, involving other agencies as appropriate.
* Lead and develop hopeful, strengths-based relationships with service users to create meaningful personal recovery plans.
* Manage own workload and help plan the workload of others; prioritise time effectively using electronic resources (e.g., diaries/scheduling).
* Protect individuals from abuse and harm in line with safeguarding policies; contribute to protection plans and appropriate information sharing.
* Coordinate protective interventions as required and work with others in safeguarding scenarios.
* Promote and model approaches that help service users develop skills to manage their own health, aligned with personal recovery plans.
* Undertake active clinical leadership and supervision within the team, covering for the manager and colleagues as required.
* Maintain the single health and social care record (paper and electronic) per professional and organisational standards.


Requirements and Preferences

* Qualified nurses with experience of working with people with acute and enduring mental health needs; recent graduates considered if they have completed preceptorship.
* Willingness to work across a wide geographical area with limited public transport access.
* Ability to work as part of a multidisciplinary team and to engage with additional services (IST, PTS, PCLS, social workers, primary care, inpatient and day services, voluntary sector).
* Commitment to evidence-based practice and personal recovery planning.
