We invite you to join our supportive team. Our service provides support to customers with mental health, learning disabilities, and dual diagnosis needs, living in our 54 properties across Blyth, Cramlington, Ashington, and other parts of Northumberland.
Our team emphasizes a person-centered approach, making each day unique. Responsibilities include assisting with daily tasks, supporting attendance at college, and helping with mental health and anxiety management. We also support customers in activities like going on holiday!
Typical day as a Support Coordinator
* Creating support plans and coordinating Support Workers.
* Helping customers identify their aspirations.
* Supporting independence in daily living activities such as cooking, shopping, budgeting, and maintaining tenancies.
* Carrying out risk assessments, support planning, and reviews.
* Engaging with customers through chats and catch-ups, fostering a collaborative environment.

You bring
* Passion for supporting customers and working collaboratively.
* Experience in creating and supporting person-centered plans.
* Experience in assessing customer referrals.
* Ability to work independently, stay calm under pressure, and be resilient.
* Willingness to travel around the community; access to public transport or a personal vehicle is preferred (expenses paid).

Job details
* Work on a rota between 8 am and 8 pm, including weekends and bank holidays.
* Flexibility is needed to meet customer needs, with support to balance work and personal life.
* Use technology for support plans, online learning, and collaboration.
* An Enhanced DBS check is required; we cover the cost.
