About the Role:

What will your responsibilities be?
The team is responsible for:
* Responding to safeguarding concerns and crisis situations.
* Conducting outcome-focused assessments and developing Care & Support Plans.
* Promoting wellbeing through theWellbeing Matters Service.
* Addressing urgent equipment and manual handling needs.
* Reassessing individuals whose needs have changed.

Qualifications & Experience:
To be successful in this role you must have,
* A recognised Social Work qualification and registration with Social Care Wales.
* DBS on the update Service
* A commitment to safeguarding, co-production, and person-centred practice.
