About the role:
* Chosen Care Group is currently recruiting Care Manager with minimum of one year experience in domiciliary / extra care service.
* You need to have level 5 in Health and Social Care or equivalent, and proven understanding of CQC regulations, inspections and what it means to achieve a ‘Good’ or ‘Outstanding’ rating.
* You should be able to draft comprehensive care plan and Risk assessment

Main duties -
* Manage the day-to-day operations of the care service, ensuring that the highest standards of care provided to all patients.
* Monitor to evaluate the quality of care provided and ensure that all patient’s needs are met promptly and efficiently.
* Carry out regular assessments of patient to identify any changes in their needs and develop appropriate care plans and update regularly.
* Train staff and ensure that all staff are adequately trained and supervised to provide the best possible care for each patient.
* Audit to maintain accurate records of all patients, care plans and related documentation
* Liaise with family, health care professionals and social workers to ensure the best possible care for each patient.
* Ensure that all relevant regulations and policies are always adhered to.
* Organise and attend special trainings along with the staffs delivered by professionals in hospitals or other settings in the community
