Responsibilities
Work with both older and younger adults who have long‑term complex challenges around physical health, mental health and emotional well‑being. Assess and support the needs of adults affected by long‑term complex and adverse conditions, as well as their carers and families, in partnership with G.P. surgeries and third‑sector partner agencies.
Eligibility Requirements
* Ability to greet customers through the medium of Welsh is a requirement.
* Protecting children, young people or adults at risk is a core responsibility of all council employees.
* Enhanced with Adults Barred list criminal records check by the Disclosure & Barring Service (DBS) is a requirement.
Working Conditions
The council's Hybrid Working Policy applies to this post. This provides a framework for establishing how you will undertake working hours between your home and the office.
