Job Description:
An exciting opportunity has arisen at Sandwell for a position within the Disabilities Team. The role involves conducting assessments of service users’ needs, strengths, and weaknesses, and developing tailored intervention plans with measurable goals. You will monitor and evaluate clients’ progress, provide crisis intervention and empathetic support, and collaborate with multidisciplinary teams including Occupational Therapy, Physiotherapy, and Intermediate Care Support Workers to promote independent living. Maintaining accurate records is also essential.

Qualifications and Experience:
Applicants must have Social Work England registration, be eligible to work in the UK, hold a full UK driving licence, and possess a degree or equivalent qualification in Social Work. Post-qualified experience is preferred.
