About The Role
Bexley Adult Social Care are looking to recruit a qualified social worker to work within the Complex Care & Review Team.
Our Social Workers within the Complex Care Teams help to provide services to vulnerable adults in our community. This includes older people and those with physical or learning disabilities or mental health problems, as we strive to help them achieve brighter, more positive futures.
Alongside other members of a multi-disciplinary team, you'll assess the service user's needs (and that of their carer), to come up with a creative and practical, programme that will enhance their quality of life. You'll then help them gain access to relevant resources, all whilst maintaining your statutory duties and documenting their progress meticulously – monitoring and reviewing their care plan as you go.
