Overview
Latham House Medical Practice is looking for an experienced Clinical Pharmacist to complement the existing team of Pharmacists and Technicians. Working alongside our current Pharmacy Team the post-holder will quickly become an integral member of the Practice and enjoy a diverse range of duties and responsibilities including:
Responsibilities
* Resolving medicines related queries with both healthcare professionals and patients.
* Further develop the Practices strong working relationships with care homes and community nursing supporting the management of patients.
* Conducting medicines/polypharmacy reviews either in a face-to-face clinical setting or by telephone.
* Developing their clinical skills in a chosen area of expertise.
* Contributing to and supporting the Audits and Quality Improvement Programme to ensure prescribing is aligned to the local formulary.
* Ensuring the achievement of targets and budgets.
* Liaising with Pharmacists working within the Leicester, Leicestershire and Rutland ICB.
* As a self-motivated and enthusiastic Clinical Pharmacist, the successful candidate will enjoy opportunities including: regular mentor discussions, face-to-face reviews, and access to a Practice-based education, learning and training opportunities.
Main duties
Work as part of a multi-disciplinary team in a patient-facing role. Help support the repeat prescription system, deal with acute prescription requests, and medicines reconciliation on transfer of care and systems for safer prescribing, providing expertise in clinical medicines advice while addressing both public and social care needs of patient in the GP practice (s). Take responsibility for areas of chronic disease management within the Practice and undertake clinical medication reviews to proactively manage patients with complex polypharmacy. Provide primary support to general practice staff with regards to prescription and medication queries. Provide clinical leadership on medicines optimisation, quality improvement, maintaining expenditure within set budgetary limits and manage some aspects of the quality and outcomes framework and enhanced services. Ensure that the practice integrates with community and hospital pharmacists to help utilise skill mix, improve patient outcomes, ensure better access to healthcare and help manage workload. The role is pivotal to improving the quality of care and operational efficiencies so requires motivation and passion to deliver excellent service within general practice. Engage, support and provide a training environment for the Team.
