Job responsibilities
What your typicalday looks like:
Beingresponsible for the assessment, implementation of treatment and thedevelopment of programs of care for designated patients and providingfeedback / escalating discussion regarding meeting patient need throughsupervision/monitoring processes.
Providingnursing care to individuals making use of current evidence to inform thedelivery and promote the development of clinically effective care.
Ensuringa continuity of care in accordance with the care programme, seekingsupport from other specialist practitioners/operational managers asrequired.
Identifying need, and providing advice,support and demonstration to the patient/carer on the use of the equipmentto ensure a personal duty of care and compliance with health and safetypolicies.
Monitoring and reporting to the seniornurse effectiveness of equipment and aids in patients homes to maximisethe use of resources to enable care packages to be delivered.
Maintaining and recording accuratehealth records in patients homes and health care settings in line withCaldecott and DHU Policies and Procedures.
To be a successfulCommunity Staff Nurse youll need:
RegisteredNurse Adult 1 / 2
Postregistration experience at Level 1.
Knowledgeof local and national priorities.
Broadclinical knowledge base.

Qualifications
* Registered Nurse Adult 1 / 2
* Post registration experience at Level 1.
* A recognised teaching/mentoring qualification OR willingness to undertake necessary training.
* Relevant academic qualifications in specialist subjects
* Full UK driving licence

* Able to work unsocial hours and in particular the overnight period, including some weekends and bank holidays.
