Are you passionate about palliative care? Do you want to work with an enthusiastic team developing your palliative care skills? An exciting opportunity has arisen for a Band 6 nurse to join the hospital palliative care team based at the Royal Sussex County and Princess Royal Hospitals. 
This role is eligible for visa sponsorship under the UK Visas & Immigration (UKVI) Skilled Worker route however sponsorship can only be provided where applicants meet the specific requirements set by UKVI, including the relevant skill and salary thresholds (£25000), and any role-specific criteria.

Main duties of the job
Working alongside the Band 7 clinical nurse specialists and Band 4 support workers, the post holder will provide holistic specialist clinical assessment, intervention, support, advice, and education to patients and carers with palliative and end of life care needs. 
The post holder will work in close collaboration with wards, clinical and multi-disciplinary teams to ensure delivery of high quality individualised care for patients and their families requiring specialist palliative and end of life care in line with local and national guidance. In particular advising and supporting clinical teams in all aspects of end of life care. The post holder will take part in the nursing rota to provide a 7 day palliative care service between the hours of 9am - 5pm at both Royal Sussex County and Princess Royal Hospitals.

Detailed job description and main responsibilities
 1. Using expert clinical knowledge and understanding act as nurse adviser to patients, carers and staff for those with palliative care/end of life needs within the hospital setting.
 2. Undertake a holistic assessment of the patients and carers needs and ensure that it is both recorded and communicated to the appropriate people.
 3. Promote an optimal environment for the delivery of care for patients within the hospital setting in partnership with other health care professionals within the trust.
 4. Facilitating the discharge of patients in accordance with the rapid discharge guidelines and the patients preferred place of care and death.
 5. Facilitate the discussion around advance care planning and medical 
contingency plans where possible and appropriate.
 6. Support the palliative care CNS team in the provision of data and reports that meet the required key performance indicators for the service in a timely manner.
