Seven Resourcing is recruiting an experienced Social Worker to join an Adults Hospital Discharge Team in Dumfries & Galloway. This role plays a vital part in supporting timely and safe hospital discharges, working closely with health colleagues to ensure adults receive appropriate care and support in the community.
Key Responsibilities
* Supporting adults through the hospital discharge process to enable safe and timely transitions from hospital to community settings
* Completing assessments under relevant legislation to determine care and support needs
* Developing and reviewing care packages in collaboration with health professionals, service users, and carers
* Managing a fast‑paced caseload with a strong focus on outcomes and discharge planning
* Undertaking safeguarding enquiries and risk assessments where required
* Working as part of a multidisciplinary team across health and social care
* Maintaining accurate and up‑to‑date case records in line with local authority standards
Requirements
* Degree or equivalent qualification in Social Work
* Registration with the Scottish Social Services Council (SSSC)
* Proven experience working within an adults hospital discharge or community setting
* Strong knowledge of relevant legislation and discharge planning processes
* Ability to work effectively under pressure and manage competing priorities
* Excellent communication and partnership‑working skills
