Job summary
The Kelvedon & Feering Health Centre are seeking to recruit a full time practice nurse at our growing practice in the village of Kelvedon & Feering. The ideal candidate will have experience of primary care nursing and able to perform a range of procedures in vaccinations, immunisations, wound care, ECG, phlebotomy, INR. A knowledge of LTC management (Diabetes, Respiratory Conditions, Asthma/ COPD, hypertension) will be an advantage.
The post-holder will be comfortable using IT. Training will be given to improve and develop your skills. The practice has plans to move to a purpose-built site soon and we encourage staff to continue their PDP and attend training as required. We are looking for a nurse clinical team lead to take the lead in infection control and who will collaborate closely with a team of supportive and collaborative clinicians to ensure the greatest level of care delivered to all patients.
Main duties of the job
1. Undertaking health assessments, administering medication and treatments, and performing diagnostic tests.
2. Providing and evaluating a range of health interventions to ensure that patients health and social care needs are met.
3. General health and well-being promotion as well as creating individual, person-centred plans for patients with, or at risk of, a long term condition.
4. Managing and treating long-term conditions, in line with national and local policies and the practice needs.
5. Reviewing medication for therapeutic effectiveness, in accordance with evidence-based practice as well as national and practice protocols; working with patients to support their adherence to prescribed treatments.
6. Prioritising health problems and intervening appropriately, to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.
7. Supporting and managing the health needs of women presenting for sexual health and contraception, and cervical cytology consultations.
8. Implementing, and participating in, vaccination and immunisation programmes for both adults and children, including patients travelling abroad.
9. Meeting the needs of patients presenting for opportunistic wound care.
10. Hypertension review and management.
11. Running asthma, COPD and diabetes clinics and reviews.
12. New patient and health checks.

17. A relevant nursing degree.
18. Current NMC registration with an active PIN.
19. Fundamentals of Practice Nursing course (General Practice Nurse Foundation Programme) or equivalent experience or qualifications;
20. Competence to undertake asthma, COPD and diabetic clinics and reviews, holding relevant certificates.

21. Evidence of clinical skills and experience in treatment room consultation, phlebotomy, ECG recording, screening, management of long-term conditions, cervical cytology sampling, vaccinations and immunisations, assisting with minor surgery, dressing and wound care, and recording spirometry.
