Who You Are You are a dedicated professional passionate about supporting young people aged 18–25 as they transition from children's to adult social care. You have a strong understanding of the Care Act 2014 and are committed to promoting wellbeing and independence for young adults in care. What the Job Involves As a member of the Young Adults (Transitions) Team, you will complete strengths-based assessments, identify eligible needs, and develop care and support plans. You will handle Section 42 safeguarding enquiries, conduct CHC and Mental Capacity Act assessments, make best-interest decisions, and contribute to court work when necessary. Collaboration with young adults, families, and multi-agency partners is essential to ensure safe and successful transitions into adult services.
