<div><p>The East Community Mental Health Team (CMHT) are looking for a skilled Senior Mental Health Practitioner to work within the services Shared Care and duty team. The purpose of this is to provide consistent and pre-planned crisis care for patients within our caseload who are experiencing a decline in their mental health and require extra support.</p><p>The shared care team are part of the multi-disciplinary team and are centre to the daily functions of the CMHT. The role covers a wide range of interventions and communication with primary and secondary care is critical in making sure that the patients are at the centre.</p></div>
