Band 7 Clinical Nurse Specialist - Respiratory Services

We are expanding our successful teams to support the Acute Respiratory Virtual Ward and are seeking an enthusiastic, knowledgeable Band 7 Clinical Nurse Specialist committed to excellence in respiratory care.
As a key member of the senior team, you will lead and develop a multidisciplinary workforce – including nurses, physiotherapists, nursing associates, associate practitioners, physio technicians and administrative staff – to deliver outstanding care for people with respiratory illness in their preferred setting.
Based at Farnham Community Hospital, our team covers NE Hampshire, Farnham and Surrey Heath, caring for patients in hospital, clinics and their own homes. You will play a vital role in improving standards of respiratory care and enjoy a varied role where no two days are the same.
The post requires flexibility to work across designated sites within the ICB, spanning both community and secondary care. Core hours are Monday to Friday, 9am-5pm, this will develop into a 7‑day service, which may involve weekend rotations and occasional unsociable hours.
Main duties of the job
The Virtual Ward is an exciting new service offering home-based care to patients who would otherwise occupy a hospital bed. Care is delivered by a specialist workforce using face‑to‑face assessments and an integrated pathway to safely support patients in their own homes. Working closely with other experts, you will work both independently and as part of a multi‑disciplinary team, providing care that is underpinned by research and innovation.
The successful candidate will provide specialist respiratory intervention, and support the multidisciplinary team in assessing and developing a holistic management plan. You will be expected to perform assessment of respiratory patients with diverse presentations and complex physical and psychological conditions, developing and delivering an individualised physiotherapy treatment programme based on this. You will understand the assessment and management of multiple respiratory conditions and be aware of local and national guidelines for COPD, Bronchiectasis, Interstitial Lung Disease, Asthma and Oxygen.

* Significant relevant clinical experience (postgraduate) in the same or related specialty
* Some recent clinical experience in the acute or respiratory setting
* Experience in managing the acutely unwell respiratory patient in the hospital setting
* Experience in acting as a clinical supervisor and mentor to others
Experience – Desirable
* Experience of applying research evidence to clinical practice
* Previous community nursing experience
Special Requirements – Essential
* Full driving licence and access to a car for use to travel across sites and perform domiciliary visits
* Willing and able to work weekends and unsocial hours
