Care Plus Group Primary Care practices provides the first point of contact for registered patients accessing the health care service. The service works collaboratively with patients to help them manage and prevent illnesses and coordinate access to specialised care.
Care Plus Group Primary Care practices, Open Door and Quayside, provide services for a client group with diverse and sometimes complex needs, including substance misuse, homelessness, mental health problems and self-neglect.
We are a social enterprise and work as a multidisciplinary team with clinicians and social staff, including mental health practitioners.

Main duties of the job Department: CPG Primary Care Base: Open Door and Quayside Medical Centre 18 hours per week (4 sessions per week), permanent, Tuesdays and Fridays Salary: £11,200 per annum per session
As a GP you will manage a caseload and address a wide range of health needs, ensuring the highest standards of care for all registered and temporary patients. You will undertake face‑to‑face and telephone consultations, and as required home visits. Day‑to‑day tasks will include reviewing results, issuing repeat prescriptions and completing appropriate paperwork. Responsibilities will reflect the NHS GP Contracts and may change as services adapt.
