Under the leadership and management of the Team Manager you will work within a team of Social Workers and other Social Care Practitioners to provide a Safeguarding Triage service to adults with health and social care needs, achieving high quality, timely and person‑centred Safeguarding concerns. You will work to improve outcomes and increase/maintain the independence of vulnerable adults.
Key Responsibilities
 * Screen and triage incoming safeguarding concerns.
 * Gather relevant information and apply the Making Safeguarding Personal approach.
 * Make threshold decisions regarding whether concerns should proceed through the safeguarding process.
 * Identify and recommend appropriate next steps for adults at risk of abuse or harm.
 * Collaborate with external agencies to assess risks and support adults to live free from abuse or harm.
 * Share expertise and support team development within a multi‑disciplinary context.
Social Care Practitioners are required to work collaboratively with colleagues from different teams, disciplines and partner organisations to provide a seamless service to individuals, families and carers. They help protect vulnerable adults from harm or abuse and support people to optimise their choices in order to live as independently as possible, working with communities, individuals and families to promote their personal strengths.
Working Pattern
08.45 – 17.15 Mon – Thursday and 08.45 – 16.45 Friday. Flexible working options are available if feasible and following a discussion with the Team Manager.
What You’ll Need To Succeed
The Social Care Practitioner will ensure they acquire and improve their knowledge and skills, contribute to the learning and development of other team members, and behave professionally at all times, reflecting the values of the department and the council.
You Will Have The Following
 * A minimum Level 3 qualification in health, social care or a relevant area, or the capacity and willingness to work towards appropriate qualifications.
 * Demonstrable skills in analysing information and the ability to translate analyses into a coherent plan and outcome.
 * The ability to work creatively under stressful conditions, know when and how to ask for support, and to pass information and cases to more experienced or qualified workers as appropriate, following negotiation with a supervisor.
 * The ability to reprioritise and be flexible to respond to urgent referrals.
