Join our District Nursing Service, which is dynamic, proactive, and committed to making a difference. We are recruiting motivated, passionate Band 5 Community Nurses interested in developing into roles such as:
* Band 6 District Nurse after completing the District Nurse Qualification.
* Chronic Condition Case Manager with disease-specific training for complex needs.
* Nurse Practitioners through modular clinical skills training.

Main duties
The successful candidate will possess excellent communication skills and provide effective clinical care within the District Nurse Team. The West Division of Betsi Cadwaladr University Health Board covers Anglesey and Gwynedd, working with partners to deliver high-quality, evidence-based care to maintain patients at home, prevent hospital admissions, and promote self-management.
You must be able to travel to patients' homes within towns and rural areas promptly. Welsh language skills are desirable but not essential; applications in Welsh are welcome.
