Job Title: QSW - Adults - Community Care (Learning Disabilities)
Pay 32LTD inclusive or 24 PAYE exclusive per hour
Job Purpose:
To deliver high-quality, person-centred social work practice in line with the Care Act 2014 and associated legislation. The post holder will manage a caseload of adults with learning disabilities and autism, undertake statutory duties, and contribute to safeguarding, risk management, and service improvement.
Key Responsibilities:
* Care Management and Casework:
* Undertake the full range of Care Act responsibilities, including assessments, support planning, reviews, and risk assessments.
* Manage a caseload of complex cases, ensuring timely and accurate record-keeping using systems such as Mosaic.
* Carry out statutory duties under relevant legislation, including the Mental Capacity Act and Deprivation of Liberty Safeguards (DoLS).
* Participate in the team duty system, responding to new referrals, emergencies, and safeguarding concerns.
* Undertake safeguarding enquiries in line with local multi-agency procedures and the principles of Making Safeguarding Personal.
* Identify and assess risk, completing risk assessments in accordance with Merton Care Management Procedures.
* Complete Care Act assessments, support planning, reviews, and risk assessments.
* Respond to referrals and emergencies via the team duty system.
Essential Requirements:
* Qualified Social Worker with current registration (e.g., Social Work England)
* Experience in adult social care, particularly with learning disabilities and autism
* Strong understanding of the Care Act, Mental Capacity Act, and safeguarding legislation
* Ability to manage complex caseloads and work under pressure
* Excellent communication, negotiation, and interpersonal skills
* Commitment to person-centred, strengths-based, and evidence-informed practice
Contract Details:
* Contract Length: 13.0 months
