We have an exciting opportunity for a Qualified Social Worker registered with Social Work England to come and join our dynamic Hospital Discharge Team within Adult Social Care. Newly qualified social workers and student social workers nearing the end of their studies will also be considered.
As a Social Worker on the team you will be working within a multi-disciplinary team, you will be responsible for supporting safe and timely discharges from acute and non-acute hospital settings.
You will have the opportunity to work with people who reside in Blackburn with Darwen in a variety of different settings, whether that be in people's own homes, in hospital or in a care home setting. You will also have the opportunity to work with other Blackburn with Darwen teams directly, such as The Home First and Reablement team and wider health professionals and community organisations.
The role will offer you a varied experience including being part of a multi-disciplinary team, supporting the social work duty function, triaging referrals we receive, having a face-to-face role on the ward, undertaking Mental Capacity assessment, chairing discharge planning meeting, best interest meetings and also undertaking Social Care assessment. The role is varied and everyday is different, which allows for continued development and increased knowledge in different areas of social work practice.
We are looking for individuals with the following skills /experience:
* Who are able to communicate and engage with people in a person-centred way to ensure they are fully involved in their own assessment and support planning.
* Who are able to work under pressure, to promote timely intervention and support.
* Who enjoy being part of a wider team with other MDT professionals, including our health colleagues.
* Who are creative, ambitious and driven when working with people.
* Experience of working within multi-professional teams with an understanding of Health and Social Care integration.
* Experience of undertaking strength based, person-centred assessments of individual's care and support needs and have high ambitions for people using services.
* Experience of key legislation in relation to Adults and applying this to practice. Including The Care Act 2014, The Mental Capacity Act 2005 and The Human Rights Act 1998.
* Are a qualified Social Worker or approaching the end of their social work course.
* Ability to work flexibly to cover a 7-day week rota, on a four-week rota basis.
* Weekend working post, working one in four weekends
