Are you passionate about wound care and helping patients achieve their best quality of life? This is an exciting opportunity to join the Sheffield Ambulatory Wound Care and Lymphoedema Service, a dynamic team dedicated to delivering high quality, evidence-based care.

What We’re Looking For:

We are seeking a motivated and organised nurse who:

• Has significant experience in community nursing or leg ulcer management.

• Ideally holds a prescribing qualification and mentorship training (desirable but not essential).

• Demonstrates excellent communication, clinical coordination, and holistic assessment skills.

• Understands referral pathways within leg ulcer management and promotes patient-centred care with a self-management focus.

• You will also be required to travel across different clinic sites; therefore, a driving license is desirable.

For those who are currently employed by the Trust as a Registered Nurse, you should instead apply through the Staff Nurse Transfer process via your line manager.

Main duties of the job

Work across four community leg ulcer clinics in Sheffield, assessing and managing patients with lower leg wounds and lymphoedema.

Provide advice and develop personalised care plans, focusing on evidence-based treatment and self-management strategies.

Manage the day to day running of leg ulcer clinics and carry out non-face-to-face clinical coordination, including handling new referrals and supporting healthcare professionals and patients remotely.

Essential criteria

1. Registered Nurse with NMC (Adult Nursing)
2. Evidence of continuous professional development

Desirable criteria

3. Independent Nurse prescriber V100/V150

Experience

Essential criteria

4. Significant relevant nursing experience
5. Experience in wound care and lower leg assessments

Desirable criteria

6. Understands the needs and importance of community nursing

Skills and Knowledge

Essential criteria

7. Must be able to demonstrate up to date knowledge and wide range of clinical skills
8. Must be competent in Leg ulcer assessment and Management
9. Must be able to promote patient self-management and education
10. Must be able to demonstrate an understanding of research and evidence based practice and clinical audit
11. Must have knowledge of nursing process and be able to apply to patients in community
