About the Role We are seeking an experienced and compassionate Complex Care Nurse to join our dedicated community healthcare team supporting children and adults with complex health needs in their own homes and community settings across Leeds and Newcastle. You will play a vital role in overseeing clinical care plans, supporting healthcare assistants, and ensuring high-quality, person-centered care is delivered to every client. Responsibilities Conduct comprehensive clinical assessments for clients with complex needs (e.g., tracheostomy, ventilation, PEG feeding, seizure management). Develop, review, and implement individualised care plans in line with best practice and CQC standards. Deliver hands-on clinical support to clients and provide clinical training to support staff in the community. Liaise effectively with multidisciplinary teams, including GPs, community nurses, therapists, and families. Monitor and audit care quality to ensure compliance with clinical governance and safeguarding policies. Participate in the on-call rota to provide clinical advice and escalation support when required. Requirements Essential: Registered Nurse (RGN / RSCN / RNLD) with active NMC pin. Minimum 2 years of post-registration experience within a complex care, community, or acute nursing setting. Proven experience in at least one of the following: Tracheostomy care Ventilation management PEG / enteral feeding Catheterisation and bowel management Seizure management and medication administration Strong leadership, risk assessment, and documentation skills. Excellent communication and interpersonal abilities. Full UK driving license and own vehicle (required for community visits). Desirable: Experience in pediatric complex care (for children's caseloads). Previous experience working in a homecare or community nursing role.
