Pay Range

This role offers a base pay range of $29.25/hr - $39.05/hr. The final salary will be based on your skills and experience.


Job Overview

The Wellness Nurse is responsible for monitoring the health and well‑being of residents, providing leadership support and assistance with resident care and services. Key duties include evaluating residents, administering medication, communicating with families and support services, and maintaining clinical quality assurance while demonstrating Sunrise Senior Living’s mission to champion quality of life for all seniors.


Key Responsibilities

* Coordinate ongoing health needs and services for residents to promote quality care.
* Document and follow through on changes in health care services and status.
* Assist the Resident Care Director (RCD) with resident evaluations at move‑in, 30‑day, semi‑annual, and status change intervals.
* Conduct monthly wellness visits for all residents.
* Monitor residents who experience falls, accidents, or incidents; document and report as required.
* Maintain resident wellness files and ensure weight and vital signs are obtained monthly.
* Communicate with families and responsible parties about residents’ medical needs.
* Contact attending physicians when necessary or upon family request.
* Support infection control practices and educate staff on proper precautions.
* Assist with maintaining medication rooms, carts, and treatment carts for cleanliness and availability.
* Provide clinical support to community team members as needed.
* Participate in quality assurance and clinical meetings, and contribute to continuous improvement initiatives.


Medication Administration

* Monitor medication and treatment profiles, ensuring accurate administration.
* Transcribe physician orders into MAR/TAR and document dates, times, and faxes.
* Audit MAR/TAR monthly and perform medication observations and skill checks.
* Act as liaison with pharmacy services for effective resident medication care.


Quality Assurance & Regulatory Compliance

* Apply the resident‑centered care model, APIE, and quality care indicators.
* Maintain knowledge of applicable federal, state/provincial, and local regulations and resident rights.
* Review physician reports for new residents and assist with order management.
* Ensure compliance with training requirements and maintain standards.

* Manage multiple priorities and demonstrate effective communication skills.
* Organize time and resources efficiently.
* Display sound judgment, problem‑solving, and decision‑making abilities.
* Teach and guide team members to deliver quality clinical care.


Qualifications

* Graduate of an approved nursing college or school.
* Current state/provincial LPN/LVN license.
* Minimum of one year experience in home health, assisted living, hospital, or long‑term care.
* Knowledge of nursing practices, assessment skills, and relevant regulations.
* Proficiency with Microsoft Office (Windows, Outlook, Excel) and Sunrise applications.
* Availability to work weekends, evenings, and flexible hours as required.

Pre‑Employment Requirements

Successful candidates must pass a drug test (excluding marijuana) and complete required medical tests (e.g., Tuberculosis Test, Physical Evaluation). COVID‑19 and Influenza vaccinations are required only as mandated by state and local laws.
