Social Care Assessor - City Wide
About the Role
As a Social Care Assessor you will be responsible for providing high quality support to adults in need of social care across Liverpool. The role is part of an integrated team that safeguards, assesses, plans support and promotes the well‑being of adults with care and support needs.
Responsibilities
* Contribute to the operational delivery of an effective Adult Social Care Service.
* Safeguard, assess, plan and review support for adults with care and support needs.
* Apply the Prevent, Reduce, Delay principles of the Care Act to promote independence and well‑being.
* Embed a strength‑based approach to assessment and support planning with desired outcomes.
* Plan and co‑design individual support plans with people.
* Ensure regular monitoring and reviews of support plans are carried out.
* Manage your own caseload while maintaining the reputation of the Department and Council and complying with national and service procedures and guidance.
