Overview
Are you a dedicated, flexible, reliable, and enthusiastic care professional with a passion for supporting people within their own homes? The Home Care Assessment and Reablement Team (HART) helps people to return home from hospital or prevent admission by supporting them to regain independence. As a HART Support Worker, you will provide short, intensive reablement support to people with disabilities and those who are frail and recovering from illness or injury, focusing on helping them to restore their optimum level of independence. You will motivate clients to identify their goals, provide positive feedback about their progress, and help build confidence. The support you provide may include daily living tasks such as washing, dressing/undressing, showering, bathing, meal preparation, domestic activities, and engaging in community activities. You will also support with identifying suitable assistive technology equipment, demonstrate its safe use, and advise on strategies for managing fatigue and pain, and preventing falls.

Responsibilities

* Motivate clients, identify goals, and give positive feedback on progress and achievements.
* Assist with daily living tasks (washing, dressing/undressing, showering/bathing, meal preparation, domestic tasks) and facilitate engagement in community activities.
* Identify suitable assistive technology equipment, demonstrate safe use, and advise on strategies to manage fatigue and pain and to prevent falls.
* Maintain accurate records and communicate effectively both verbally and in writing.
* Attend required job-related training and work in accordance with anti-discriminatory care standards.
Qualifications
* Hold a Level 2/3 Diploma in Health and Social Care (or other relevant equivalent qualification) or an equivalent level of relevant experience.
* Be committed to delivering high standards of care in an anti-discriminatory manner.
* Be able to work effectively on your own as well as part of a team.
* Have basic IT skills sufficient to write and maintain records.
* Be able to communicate effectively both verbally and in writing and record relevant and accurate observations.
* Be able to work under pressure and remain calm in crisis situations.
* Be willing to attend relevant job-related training as required.

Working hours and benefits
You will be required to work shifts on a rota that operates between 7:00am and 10:00pm, including evenings, weekends, and bank holidays. A DBS enhanced check for a regulated activity is required for this post.
