We are seeking two dynamic and motivated individuals to join our expanding team. As part of the CCOT, you will work autonomously to support ward-based teams in managing acutely unwell patients, playing a vital role in improving patient safety and clinical outcomes beyond the critical care setting.
About the Role:
1. Full-time substantive post (minimum 30 hours/week)
2. Participation in an extended hours and weekend rota
3. City-wide working across Sheffield Teaching Hospitals sites
4. Supported enrolment onto a Master’s-level Trainee Advanced Clinical Practitioner (ACP) programme
5. Dedicated study time for academic work alongside clinical duties

Main duties of the job
As a Trainee ACCP in CCOT, you will work autonomously to deliver high-quality, individualised care to acutely ill and deteriorating patients outside of the critical care unit.
Clinical Responsibilities:
11. Perform comprehensive A–E assessments, create and implement care plans to prevent deterioration.
12. Monitor interventions, adjusting as needed for optimal patient outcomes.
13. Provide expert advice and clinical support regarding critical care.
14. Participate in the hospital cardiac arrest team as a clinical leader.
15. Support post-critical care patients, offering both physical and psychological follow-up and appropriate referrals.
Education and Training:
16. Deliver trust-wide education, including bedside teaching and Deteriorating Patient Study Days.
17. Promote best practices and contribute to ward staff development.
Service Development and Collaboration:
18. Represent and promote CCOT across the Trust, building strong relationships with ward teams.
19. Collaborate with multidisciplinary teams to ensure cohesive patient care.
20. Engage in service improvement and development efforts within CCOT.

Working Hours: The role currently covers 08:00–18:00, seven days a week. Be prepared for future 24/7 coverage and cross-site work as the service expands.

Detailed job description and main responsibilities
The Trainee Advanced Clinical Practitioner will be working towards becoming an autonomous, accountable practitioner who will work on their own initiative and in collaboration with medical, nursing, and allied health professional colleagues as required Until signed off as competent, the Trainee ACP will work under the supervision of qualified clinician whilst developing the skills that enable them to become an autonomous practitioner. The role will be structured around the four core functions of advanced practice:

Education and Qualifications
Essential criteria
27. Registered Nurse with Valid NMC registration or Health Care Professional and registered with HCPC with relevant clinical experience
28. The candidate must be eligible to apply for and willing to complete a full MSc in Advanced Clinical Practice. Eligibility criteria includes an Honours degree at 2.2 or above in a related health care subject. Alternatively, evidence of completion of pre-entry assessment at HEI or successful level 7 study will also be considered.
29. Evidence of relevant continuous professional development
30. Advanced life support qualification (ALS/ILS) or willingness to undertake and successfully complete
Desirable criteria
31. Where possible study is funded via the apprenticeship route. If undertaking Apprentice programme, the candidate must be able to demonstrate GCSE English and Maths at C or above or functional skills assessment level 2. If unable to evidence the trainee must complete the functional skills assessment by the end of the Master’s Programme (apprenticeship requirement).
32. Post Graduate qualification in Advanced Practice
33. Advanced Physical Assessment module, Non-Medical Prescribing (if part of proposed role)
34. Recognised Teaching & Assessment Qualification or equivalent experience
35. Recognised Leadership Award/qualification or equivalent experience

36. Experience of delivering training / education to others
37. 3 years relevant post registration experience (HEI requirement)
38. Evidence of multi-disciplinary team working
Desirable criteria
39. Experience of working in an extended role with experience of autonomous practice
40. Experience of undertaking service improvement activity
41. Evidence of contribution to audit and research programs
42. Clinical / managerial leadership experience, including line management of staff, e.g. sickness/absence, recruitment/selection

47. In depth knowledge of pathophysiology of common conditions seen in the speciality and their management
48. Ability to order and interpret appropriate clinical investigations
49. Excellent presentation skills

51. Able to analyse and synthesise patient information to formulate appropriate diagnoses and management plans
