Health and Wellbeing Practitioner

The Health and Well-being Hub sits within the Targeted Youth Support Service. AfC Early Help offer is based on strong evidence best practice and is underpinned by the assessment of need and locality model of working. The Early Help directorate enables effective partnership working, evidence based practice aligned to our practice model, and supports locally commissioned services across our geographical areas.

    As a Health and Wellbeing Practitioner for Achieving for Children, you will be responsible for:

  • This role will work directly with young people who have a problematic issue with substances who require specialist 1:1 intervention, you will manage a caseload of complex young people as part of a multi disciplinary team.
  • You will need a good understanding of safeguarding, the recognition of safeguarding concerns, re appropriate recording and information sharing.
  • The accurate recording of information across various platforms, in a timely manner.

    About you

  • A Level 4 qualification (or above) in a relevant discipline, such as Youth Work, Social Care, Education, Children and Young People, or Substance Misuse, alongside a commitment to continuous professional development.
  • Experience building positive, trusting relationships with vulnerable young people, particularly those who have experienced adversity, trauma, offending behaviour, exploitation, substance misuse, gang involvement or episodes of going missing.
  • Experience supporting young people involved with substance misuse services, youth justice, leaving care and/or looked after services.
  • Strong partnership and multi-agency working skills, with experience delivering effective interventions, advocating for young people and working collaboratively with families and professionals to achieve positive outcomes.
  • A flexible, proactive and inclusive approach, underpinned by a sound understanding of relevant theory, evidence-based practice, equality, diversity and safeguarding principles.
