<div><h3>Experienced Practice Nurse (Upper Gordon Road Surgery)</h3><p>Upper Gordon Road Surgery is seeking a motivated and compassionate Practice Nurse to join our friendly and supportive team. The successful candidate will play a key role in delivering high‑quality patient care, supporting chronic disease management, and promoting health and wellbeing within our local community and supporting the wider multidisciplinary team.</p><h3>Main duties of the job</h3><ul><li>Deliver high standards of nursing care to patients within the practice setting</li><li>Undertake chronic disease management including asthma, COPD, diabetes, and hypertension reviews</li><li>Carry out cervical cytology (smears) in line with national screening programmes</li><li>Administer immunisations and vaccinations (including childhood and travel vaccines)</li><li>Perform wound care, dressings, and removal of sutures</li><li>Conduct health promotion and lifestyle advice clinics (e.g. smoking cessation, weight management)</li><li>Carry out NHS health checks and screening programmes</li><li>Assist in infection prevention and control, including maintaining clinical standards and audit processes</li><li>Maintain accurate, contemporaneous patient records in accordance with NHS and practice policies</li><li>Support GPs and the wider multidisciplinary team in delivering patient care</li><li>Participate in clinical audits, quality improvement initiatives, and practice meetings</li><li>Ensure safe management of clinical stock, vaccines, and equipment</li><li>Contribute to safeguarding processes for vulnerable adults and children</li></ul><h3>About us</h3><p>Surrey Heath Community Providers Ltd is a federation of 7 GP practices across 10 sites, serving over 104,000 patients across Surrey Heath. There may be, on occasion, a requirement to carry out other tasks, dependent on workload and staffing levels.</p><h3>Person Specification</h3><h3>Experience</h3><ul><li>Experience of working with healthcare professionals and/or previous experience in the NHS or social care</li><li>Understanding of local and national NHS policy</li><li>Understanding of Clinical Governance within primary care</li><li>Previous experience in cervical cytology screening</li><li>Previous experience working as a practice nurse in primary care</li></ul><h3>Qualifications</h3><ul><li>4 GCSEs (C and above) including English and Maths or equivalent</li><li>NMC Registration</li><li>Up‑to‑date training in immunisation and cervical screening</li><li>Postgraduate diploma or degree in primary care</li></ul><h3>Skills</h3><ul><li>Competent in treatment room procedures: wound care, ECGs, phlebotomy, immunisations and cervical screening</li><li>Managing patients with long term conditions such as: diabetes, asthma, COPD, hypertension</li><li>Highly effective communication skills, both written and verbal</li></ul><h3>Disclosure and Barring Service Check</h3><p>This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.</p></div>
