Anexciting opportunity has arisen for an enthusiastic, well-motivated andflexible Occupational Therapist to join our Urgent Community Response,Discharge to Assess and Virtual ward Services across Sunderland.
Thisrole is based with the Recovery at Home team based at Leechmere IndependentLiving Centre in Sunderland.
Theteam consists of an extensive MDT which includes nurse practitioners,occupational therapists, physiotherapists, health care assistants, therapyassistant/exercise practitioners and administrative staff. The team also worksclosely alongside partner agencies including those from the local and voluntarysector.
Main duties of the job
Thisrole would involve assessing people in their own homes who are either acutelyunwell or have recently been discharged from hospital and require enhancedintervention and short term support for their physical and functional healthneeds to remain at home.
Theservice is fast paced and you would be expected to see a wide range of patientswho present with a variety of conditions.

Toprovide care to people in their own homes to prevent admission/ readmission tohospital by providing rapid assessment of patients in the community over a 24hour period, over seven days.
Undertakecomprehensive holistic assessment of patients in both planned and unplannedsituations, be able to prioritise clinical need, initiating assessmentincluding risk assessment and implement a plan of care reflecting evidencebased guidance.
Tomaintain agreed standards of care, integrity and dignity of all patients in anenvironment which is conducive to caring for those patients in their own homes
Totriage and prioritise patients referred for therapy, coordinating and planningvisits as indicated.
Work collaboratively with multi professional teams to ensure practiceis efficient, effective and evidence based
Todevelop his /her scope of professional practice in order to support the team tomeet service requirements in accordance with professional guidelines
