Responsibilities

* Carry out Medicines Reconciliations for new patients, care home patients, and patients recently discharged from hospital; update patient records and raise queries with a GP or prescriber as needed.

* Understand the Leicestershire Shared Care Protocol system and work within it, communicating with Secondary Care colleagues as needed for Shared Care prescribing.
* Support antimicrobial stewardship and optimise local antibiotic prescribing.
* Assist practice medication teams with patient contacts regarding medication changes; liaise with Community Pharmacies for out-of-stock item alternatives and communicate changes to staff or patients.
* Respond to queries from patients and practice staff regarding acute and repeat prescriptions in a timely, professional manner.
* Provide technical and administrative support to optimise prescribing processes, reduce administrative burden for prescribers, and reduce medication wastage.
* Provide on-the-job or scheduled training to help teams develop; promote Electronic Repeat Dispensing and online ordering.
* Develop strong working relationships with PCN clinical pharmacists, practice administrative teams, and other MDT members to support the PCN’s development and meet indicators (DES/IIF).
* Highlight changes in prescribing policy or guidance and help practices implement necessary changes.
* Understand local Prescribing Quality (QIPP) schemes, QOF targets, and other local enhanced services affecting prescribing policies.
* Engage in prescribing audits or Quality Improvement projects as identified by the practice or PCN/Federation; be aware of CQC expectations regarding prescribing policies.
* Use practice clinical systems to perform prescribing-related searches and extract/manipulate data as needed.

Qualifications

* Registration with the General Pharmaceutical Council (GPhC) as a Pharmacy Technician is required.
* Commitment to undertake the 18-month Primary Care Pharmacy Education Pathway (PCPEP) unless already held or exempted by CPPE.
* A working knowledge of primary care and GP clinical systems is advantageous; training will be provided as needed.

Personal development & attitude

* Take responsibility for personal development and seek opportunities for learning and professional development.
* Support colleagues and promote a culture of sharing good practice; attend clinical supervision sessions regularly.
* Promote the work of the PCN and contribute to its development as a collaborative organisation.
* Maintain patient confidentiality and comply with Information Governance policies.
* Demonstrate flexibility to adapt to changing circumstances within a developing network.
