The role
* As a social care assessor, you will join the City‑Wide Mental Health Assessment and Care Delivery Service, delivering statutory social care responsibilities to adults experiencing mental health difficulties.
* You will undertake professional interventions with adults who have a wide range of care and support needs arising from mental health needs, promoting recovery, autonomy, and least‑restrictive practice.
* You will work closely with the multi‑disciplinary mental health team (MDT) contributing to safe, timely, and well‑coordinated assessment / support planning within the community.
* You will apply a strengths‑based, person‑centred approach when completing Care Act (2014) assessments, MCA / BIA (2005) and reviews and coordinating Section 117 aftercare.
* You will co‑produce care and support plans alongside individuals, carers, and partner agencies, promoting inclusivity, collaboration, and innovative practice that leads to sustainable community‑based outcomes.
* You will be professionally accountable for complex decision‑making and risk management across your caseload, using evidence‑based frameworks, critical reflection, and analysis to inform safe and defensible practice.
* You will demonstrate the ability to effectively manage a varied caseload, while upholding professional standards, maintaining the reputation of the Department and Council, and ensuring compliance with legislation, statutory guidance, and local policies and procedures.
